Forms for School Personnel

Clock Hours
Each mentoring teacher may receive, at no charge, continuing education clock hours for supervising a WWU intern.  The below form must be completed to receive these credits. 

Continuing Education Credit Hours:  Complete Section 1 with name, address, etc., print and then sign your name at bottom of Section 1.  Office of Field Experiences will fill out section 2.  Retain a copy for your records.

Honorariums Each mentoring teacher receives an honorarium for hosting a WWU intern.  In general, a quarter-long internship honorarium is $150 and a semester-long internship honorarium is $200.  The below forms (A-19 and W-9) must be completed to receive this honorarium.  A sample A-19 is provided for use as a guide.  The W-9 only needs to be submitted once, as it will be kept on file for future use.

Instructions: A19 – Honorarium Form

1. In the “Vendor or Claimant” box:  legal name and home mailing address;
2. Below the “Vendor or Claimant” box:  your social security number; and
3. Sign on the line next to “BY” below the description of the “Vendor’s Certificate.”

W9 – Honorarium Form

1. Legal name and home mailing address.
2. Social Security Number.
3. Sign at the bottom of the form.
