Forms for School Personnel

 

Clock Hours

Each mentoring teacher may receive, at no charge, continuing education clock hours for supervising a WWU intern.  The below form must be completed to receive these credits.  The instruction sheet contains directions on how to complete and submit the form.

Honorariums

Each mentoring teacher receives an honorarium for hosting a WWU intern.  In general, a quarter-long internship honorarium is $150 and a semester-long internship honorarium is $200.  The below forms (A-19 and W-9) must be completed to receive this honorarium.  The instruction sheet contains directions on how to complete and submit the A-19.  A sample A-19 is provided for use as a guide.  The W-9 only needs to be submitted once, for it will be kept on file for future use.
